What is an Employee Report of Wages and Physical Condition, Employee Verification of Employment, and Employee Report of Benefits for Offsets?

While you are on Pennsylvania workers’ compensation, the PA Workers’ compensation act provides insurance companies with the entitlement to find out if you are receiving income or specific benefits. These types of benefits can include wages, unemployment compensation, social security, pension, or severance pay.  If you start to receive benefits or wages, you are required to complete an Employee Report of Wages and Physical Condition as well as an Employee Report of Benefits for offsets and send it to the insurance company within 30 days of receiving your benefits or wages.

It’s important to know that if you are currently receiving Social Security Disability benefits, your compensation will not be reduced because they are not Old Age Social Security Retirement Benefits. It’s also required that you update the insurance company every consecutive six months by completing and sending the forms again. Failing to do so can result in you being charged with fraud. The workers’ compensation insurance company can also send you Employee Verification of Employment, Self-Employment or Change in Physical Condition forms every six months. These forms must be completed and returned to the insurance company within thirty days of receiving them. Failing to do so can result in termination of your workers’ compensation benefits until the forms are completed and returned.
